If you have been thinking about home care for an individual or yourself or just to be better organized, read this manual to help you determine the scope of the project and how you can do it.
There are two manuals that are used to organize and track personal information:
The PERSONAL DATA FOLDER, that contains the daily tracking and scheduling activities.
The CAREGIVERS MANUAL, a detailed text book that contains information and instructions for data collection.
The center piece of home care is the Personal Data Folder or manual. It contains an appointment calendar to organize your daily activities. The Caregivers manual is used as a text book to guide you through the start up process. Think of the Caregivers Manual as a dictionary or cook book to select the items to help organize your care. You should ask your health care professional for suggestions on what information you should track. Copy and use forms as needed.
Remember, your health is very important too, both physical and spiritual, so allow time in the schedule to attend to your needs.
It took me many years to learn how to care for an individual and put this manual together. You have the advantage of learning from my trials; these are things that worked for me.
Even if you are doing this for yourself, it will help a lot.
Section 5 Personal Data Folder (Sample Selections)
To start the home care process, go to section 5 of the caregivers manual select (QUICK START) and make an appointment calendar. This will be the start of record keeping for a Personal Data Folder for home care.
When this is done, go to Section 1 and make a traveling folder, this is very important too.

HOW TO MAKE A NEW PERSONAL DATA FOLDER
The Home Care Persona Data Folder is the working part of being a care giver. If you wondered what to do with all of the information and forms contained in the CAREGIVERS HOME CARE MANUAL, this is where you use it.
You should use this folder every day to track appointments, monitor health conditions and enter medical information in the Data/Record forms. A manual can be made to hold up to three years of appointments and Data/Record forms. The various sections have divider sheets to be used for ease of access to locate data entry forms. Forms that are in a protective cover sheet will extend the width about 1/4inch beyond the normal sheet and will act as a TAB.

HOW TO MAKE A NEW PERSONAL DATA FOLDER
A personal data folder is made up of 3 parts:
1. Cal-future forms for calendar pages by year.
2. Data/Record forms to enter and collect specific data.
3. Index and divider pages. THIS ALLOWS A CROSS REFERENCE TO THE DATA (Optional as needed to organize data.).

All new manuals start with a new 1” 'view” type 3 RING binder, plus clear sheet protectors as needed. If you are to have pages printed by someone else , a USB Drive is needed (also called a thumb drive) to copy files to be printed.
THERE ARE THREE STEPS IN STARTING A NEW MANUAL:
1 The first step is getting the appointment calendar pages (CAL-FUTURE YEARS) file, for each year of the new manual. Determine how many CAL-FUTURE YEARS and part years you want.
2 The second step selecting the Data/Record FORMS to collect the specific data you need for each year. Make a List the data/record forms you need to track your data, by each form name and the number of sheets for each; save for later use. This can be done later if not needed now.
3 The third step is manually selecting the INDEX AND DIVIDER FORMS for the period desired; 3 years or 1 year.. These forms are optional and used as an aid in locating the various manual parts. They are a cross reference aid to the data.
QUICK START GUIDE
All files and links mentioned here can be found on the “List of Pages and Index sheets” page in Section 5.

Select the years you want; links to year files are found in the “Cal-Future Year list” page.

Select any or all of the Data/Record files you want, found in the “Data/Record Copy List and Links" page .

Select “3 YR forms” or “1 YR forms” for divider sheets.
This will provide a three year personal records set of files; a single data form for bowel movements and vital signs; and index and divider pages for three years. Print your items or copy to a USB Drive to be printed later.
